WI Small Employer Uniform Employee Application
Effective 5/1/04, all carriers must use this form for employee enrollments for new Small Employer cases requesting medical coverage. This will be the only form accepted for new case enrollments signed May 1, 2004 and later that include medical.

PRINCIPAL OFFERS PACKAGE DISCOUNT
The principal is offering a discount when three or more qualifying coverages are written. This applies to new and in force cases. Contact the Appleton or Milwaukee RBG office for details on what coverages qualify and get the discount.
